DOL Extends Expiration Date for Disability Self-Identification Form

The U.S. Department of Labor (DOL) has extended the expiration date of the Voluntary Self-Identification of Disability Form (CC-305) through July 31, 2029.

The form, maintained by the Office of Federal Contract Compliance Programs (OFCCP), is used by covered federal contractors and subcontractors to invite applicants and employees to voluntarily identify whether they have a disability. The form is part of the requirements under Section 503 of the Rehabilitation Act, which requires covered federal contractors to take affirmative action to employ and advance qualified individuals with disabilities.

What Employers Should Do

Federal contractors should:

  • Continue using the CC-305 form for required self-identification.
  • Confirm that recruiting and HR systems are using the correct version of the form.
  • Continue to invite applicants and employees to voluntarily self-identify as required.
  • Ensure self-identification information remains confidential and is not used in employment decisions.
  • Review processes to ensure employees are invited to re-self-identify at least every five years.

The current form states that completing the survey is voluntary and that the information is confidential and will not be provided to individuals making hiring decisions.
