This sample communication can be used to inform an employee that the information on his/her W-2 wage and tax statement does not match the name and/or Social Security number on file with the Social Security Administration. Employers should treat all SSN mismatch letters similarly and follow the same process regardless of an employee’s race, national origin, or identification documents provided at the time of hire. Receipt of a mismatch letter alone does not necessarily indicate the employee is unauthorized for employment.
