This form can be used to track and monitor an employee's approved accommodation. Employers should maintain regular follow-up with accommodations after they are in place. In some cases, an accommodation stops being effective for various reasons such as: the employee’s limitations change, workplace equipment changes, the job changes, the workplace changes, or the accommodation becomes an undue hardship for the employer. This form should be modified each time with information specific to each case.
