NOTE TO EMPLOYERS: The following sample policy should be modified to fit the organization and can be included within a bereavement or FMLA policy. If necessary, documentation to verify the need for this leave may include a death certificate, published obituary, or written documentation of death, burial, or memorial services from a mortuary, funeral home, burial society, crematorium, religious institution, or government agency. Also, keep in mind that employees may elect to substitute paid time off for this leave but are not required to do so.
